GENERAL INFORMATION
If you are a new patient, you should bring the list of all your medications printed out by your pharmacy or call them to fax us the list.
For all new patients, a legible medical summary from your family medical doctor is obligatory.
Also, if you have been undergoing some investigations, it is very important that you provide us copies.

Please fill up the pertinant questionaires( documents) before coming to our clinic.
